Job Responsibilities:
  • Strategic Leadership: Develop and implement operational strategies that enhance profitability and efficiency across the company's chronic care programs.
  • Revenue Management: Drive the profitability of chronic care management services, including budgeting, forecasting, and resource allocation to ensure financial targets are met.
  • Program Development: Oversee the design and enhancement of clinical programs to meet the evolving needs of patients and to comply with regulatory standards.
  • Customer Service Oversight: Ensure exceptional service levels are maintained across all customer touchpoints. Develop strategies to enhance patient engagement and satisfaction.
  • Team Management: Lead, mentor, and develop a high-performing team of managers and healthcare professionals dedicated to chronic care management.
  • Performance Metrics: Monitor and report on operational performance metrics, making data-driven decisions to optimize processes and outcomes.
  • Stakeholder Engagement: Collaborate with internal and external stakeholders to ensure that operational initiatives are aligned with overall business objectives.
Education Necessary for Position:
  • Bachelor's degree in Software Development, Healthcare Management, Administration, or related field
Experience & Skills Necessary for Position:
  • Minimum of 10 years' experience in a senior management role in a healthcare setting, with a strong emphasis on chronic care management.
  • Extensive experience in managing clinical programs, customer relations, and team leadership.
  • Demonstrable track record of improving service delivery and achieving financial success within a healthcare operation.
  • Strong leadership skills with the ability to drive team performance and foster a culture of excellence and accountability.
  • Excellent analytical and problem-solving abilities, with a keen focus on operational efficiency and strategic development.
  • Proven ability to manage and oversee large budgetary operations in a healthcare setting.
  • Strong interpersonal and communication skills, capable of effectively negotiating and resolving conflicts.
  • Deep understanding of chronic care management and the healthcare industry, including regulatory and market dynamics.
  • Ability to innovate and integrate technology and business solutions into chronic care practices.
  • Highly organized, detail-oriented, and focused on delivering results
  • Passionate about delivering an exceptional client experience and dedicated to client success
  • Prior CCM experience is a must


Work Environment:

This position operates in a professional office environment. On-site, Monday-Friday 8am - 5pm.

Pay: $120,000.00 per year
