Become a vital member of our healthcare team as an Advanced Practice Provider in Post-Acute Care

We are looking for a committed Nurse Practitioner or Physician Assistant specializing in Family Medicine, Internal Medicine, or Geriatric Medicine to enhance our post-acute care services.

This is a Full-Time role, offering a 4-day work week with a flexible schedule that empowers you to choose your working hours.

Key Responsibilities:

  • Deliver primary healthcare in a post-acute environment for both short-term and long-term patients
  • Oversee chronic conditions, diagnose health issues, and develop comprehensive care strategies
  • Request and analyze laboratory results
  • Prescribe and oversee medication regimens
  • Work collaboratively with physicians and other healthcare team members
  • Provide education to patients and their families on managing chronic health issues
  • Foster strong connections with patients, their families, and facility personnel
  • Encourage and promote healthy lifestyle practices

Qualifications:

  • New graduates are encouraged to apply
  • Experience in primary care or post-acute care is advantageous
  • Must possess an active and unrestricted Nurse Practitioner or Physician Assistant license in Washington state
  • DEA registration is necessary

Compensation and Benefits:

  • Attractive compensation model based on productivity
  • Salary range from $93,000 to $114,000
