The focus is on person-centered care when coordinating discipline visits, as well as ensuring compliance with regulatory bodies. The individual in this position will collaborate with field clinicians, families, patients, facilities and assist the Hospice Clinical Director, or designee as needed. This position will be split between our Ridgefield and New Milford Offices. Responsibilities: Maintains patient schedules assuring all discipline daily visits are assigned. Reconciles patient visits against visit orders to ensure regulatory guidelines are followed. Ensures all patient re-certifications are scheduled and completed. Coordinates with families, patients, and clinicians to ensure compliance and high level of satisfaction. Communicates with patients notifying them of scheduled appointments or changes in appointments as needed. Corresponds with clinical field staff as needed. Identifies problems and potential problem situations and escalates to Hospice Clinical Director or designee. Assists Hospice Intake Coordinator in their role and duties and is dually trained in both roles. Assists with obtaining physician signatures and faxing complete orders to facilities. Manages soft supplies, including ordering from vendor and gathering patient orders proficiently. Acts as a back-up or support to other roles as time allows/as needed. Is computer literate, enters information into computer in a timely and accurate manner. Participates in total quality improvement process for both agency and self. Provides feedback focusing on performance improvement of RVNA. Knowledgeable of RVNAHealth policies and procedures, programs and services, and represents RVNAHealth in a positive, professional manner. Qualifications: A minimum of two years of healthcare experience, including medical terminology. Bachelor’s degree or licensed or experienced high-level functioning Medical Assistant, CNA, Coder, or other healthcare personnel. Preferred, minimum one year of hospice experience with a strong knowledge base of home health or hospice and associated regulations. Ability to handle several situations simultaneously in a professional, calm, dependable manner. Exceptional customer service skills including phone manner and ability to build relationships in a fast-paced environment. Is computer literate, enters information into computer in a timely and accurate manner. Knowledgeable of sales theories and practices.
