Role Overview:

The Hospice Patient Service Coordinator plays a crucial role in managing hospice patient visits according to established orders, excluding certified nursing assistants. This position is vital in addressing scheduling inquiries and ensuring an exceptional customer service experience.

The focus is on delivering person-centered care while coordinating visits across various disciplines and ensuring adherence to regulatory standards.

Key Responsibilities:

- Maintain patient schedules to ensure all discipline visits are assigned daily.
- Reconcile patient visits with visit orders to comply with regulatory guidelines.
- Schedule and complete all patient re-certifications.
- Collaborate with families, patients, and clinicians to uphold compliance and satisfaction levels.
- Communicate with patients regarding scheduled appointments or any changes.
- Correspond with clinical field staff as necessary.
- Identify and escalate potential issues to the Hospice Clinical Director or designated personnel.
- Support the Hospice Intake Coordinator and be trained in both roles.
- Assist in obtaining physician signatures and faxing complete orders to facilities.
- Manage soft supplies, including vendor orders and patient order collection.
- Provide backup support to other roles as time permits.
- Demonstrate computer literacy by entering information accurately and promptly.
- Engage in quality improvement processes for both the agency and personal development.
- Offer constructive feedback aimed at enhancing RVNA's performance.
- Stay informed about RVNAhealth policies, procedures, programs, and services, representing the organization positively.

Qualifications:

- Minimum of two years of experience in healthcare, with familiarity in medical terminology.
- A Bachelor's degree or equivalent experience as a high-level Medical Assistant, CNA, Coder, or other healthcare professional.
- Preferred: At least one year of hospice experience with a solid understanding of home health or hospice regulations.
- Ability to manage multiple situations simultaneously in a calm and professional manner.
- Exceptional customer service skills, including effective communication and relationship-building in a fast-paced environment.
- Proficient in computer skills, ensuring timely and accurate data entry.
- Knowledgeable in sales theories and practices.
