Job Summary

We are seeking a highly skilled and experienced Patient Care Manager to join our team. As a Patient Care Manager, you will be responsible for the day-to-day operations of our hospice sites, ensuring the delivery of high-quality patient care and services.

Key Responsibilities

  • Administer the clinical aspects of the hospice program, ensuring the provision of quality care to patients.
  • Manage and oversee clinical and administrative functions, including recordkeeping and staff supervision.
  • Respond to phone referrals and inquiries in a timely and professional manner.
  • Participate in on-call nursing schedules and provide direct skilled bedside nursing care in patients' homes.

Requirements

  • Registered Nurse with a minimum of Associate's Degree in Nursing required from an accredited school of nursing.
  • Three years' experience in direct patient care in a clinical setting required.
  • Minimum one year of management/supervisory experience required.
  • Two or more years' experience in hospice or home care required.
  • Current license to practice nursing in the state of requested employment.

Personal Qualities and Skills

  • Sensitivity to the feelings of the terminally ill and their families.
  • Ability to demonstrate flexibility and perform a variety of tasks.
  • Effective communication and interpersonal skills.
  • Ability to influence people and make generalizations and evaluations based on sensory or judgmental criteria.

Specialized Knowledge and Skills

  • Knowledge of terminally ill patients and their families, as well as the hospice concept.
  • Understanding of hospice services and roles of all disciplines providing hospice care.
  • Excellent patient assessment skills and good oral and written communication.
  • Thorough knowledge of managed care principles, regulatory guidelines, and management principles.
