We are seeking a dedicated and empathetic Registered Nurse to become a vital part of RVNAhealth. In the role of Continuous Care Nurse for Hospice, you will be tasked with delivering direct patient care, providing palliative nursing services, and offering bereavement support. This role demands exceptional clinical expertise, meticulous attention to detail, and a steadfast commitment to delivering high-quality patient care.

Key Responsibilities:

  • Deliver direct patient care as designated, ensuring findings are communicated to the RN Case Manager
  • Provide palliative nursing services that integrate psychosocial, cultural, and biological aspects
  • Administer medications, manage symptoms effectively, and foster relationships with patients and their families
  • Collaborate with the Interdisciplinary Group (IDG) to maintain continuity of care
  • Accurately document observations and interventions
  • Engage in patient counseling and educational activities
  • Communicate proficiently with team members, patients, families, and healthcare providers
  • Proactively identify and address potential issues
  • Assist with dietary counseling and other supportive functions as required

Qualifications:

  • Graduate from an accredited nursing program
  • Registered Nurse license in the State of Connecticut
  • A minimum of two years of clinical experience
  • Preferred background in hospice or home care settings
  • Strong organizational skills and ability to multitask
  • Excellent customer service skills and proficiency in computer use
