The primary role of the Hospice Patient Service Coordinator is to manage hospice patient appointments according to directives, excluding certified nursing assistants, address scheduling inquiries, and facilitate an exceptional customer service experience.

The emphasis is on person-centered care while coordinating visits across various disciplines, ensuring adherence to regulatory standards.


This position involves collaboration with field clinicians, families, patients, and facilities, while also providing support to the Hospice Clinical Director or their designee as required.

This role will be shared between our designated offices.

Key Responsibilities:
  • Maintain patient schedules to ensure all discipline visits are appropriately assigned.
  • Reconcile patient visits with visit orders to guarantee compliance with regulatory requirements.
  • Schedule and complete all patient re-certifications.
  • Coordinate with families, patients, and clinicians to ensure compliance and satisfaction.
  • Notify patients of scheduled appointments or any changes as necessary.
  • Communicate with clinical field staff as needed.
  • Identify and escalate potential issues to the Hospice Clinical Director or designee.
  • Assist the Hospice Intake Coordinator and be trained in both roles.
  • Facilitate the acquisition of physician signatures and ensure orders are sent to facilities.
  • Manage soft supplies, including vendor orders and patient order collection.
  • Provide backup support to other roles as time permits.
  • Demonstrate computer proficiency by entering information accurately and promptly.
  • Engage in quality improvement processes for both the agency and personal development.
  • Offer constructive feedback aimed at enhancing RVNA's performance.
  • Understand RVNAhealth policies, procedures, programs, and services, representing the organization positively and professionally.

Qualifications:
  • A minimum of two years of experience in healthcare, with familiarity in medical terminology.
  • A Bachelor's degree or equivalent experience as a high-level Medical Assistant, CNA, Coder, or other healthcare professional.
  • Preferred: At least one year of hospice experience with a solid understanding of home health or hospice regulations.
  • Ability to manage multiple situations simultaneously in a calm and professional manner.
  • Exceptional customer service skills, including effective communication and relationship-building in a dynamic environment.
  • Proficient in computer use, ensuring timely and accurate data entry.
  • Knowledgeable in sales theories and practices.
