We are seeking a **Client Engagement Specialist** to help support the work we do.

**Position Objective**:
The Client Engagement Specialist provides comprehensive and quality home-based services to clients with HIV/AIDS residing in Chicago. In addition to managing relationships with clients, assessing client needs, and providing case management, the Client Engagement Specialist will collaborate with the Community Health team and other program staff to ensure the multiple needs of clients will be met.

**Responsibilities**:

- Develop and implement treatment/service plans for all clients accessing services.
- Provide case management on an individual basis to clients, including linkages to supportive services, treatment and care.
- Conduct home visits to the clients on a regular basis and maintain telephone contact as required.
- Assist program participants to ensure they receive the necessary HIV care and resources. Supporting program participants in making positive health choices.
- Collaborate closely with other HIV/AIDS program staff and community staff to provide holistic services for clients.
- Manage administrative and program responsibilities, including writing of progress notes, evaluations, and reports in an accurate and timely manner to meet confidentiality and compliance standards.
- Manage case files, case-noting, and other administrative tasks efficiently.
- Attend professional trainings each year and adhere to all current DRS’ policies and procedures as prescribed by the Agency and funder.
- Actively participate in team meetings and program planning meetings.

**Qualifications (Skills, Knowledge, & Experience)**:

- Requires a minimum of a Bachelor’s degree in social work or social sciences.
- Licensed social workers and certified case managers a plus.
- Possess a genuine concern for the well-being of others and the ability to empathize with their life situations.
- An understanding and appreciation of diverse identities, cultures, beliefs, and practices.
- Maintain a high level of professionalism, including integrity, ethics, and respect for confidentiality.
- Effective speaking and written communication skills while interacting with clients, staff at other agencies, and co-workers.
- Excellent observational skills and ability to accurately describe client situations.
- Familiarity with Microsoft Office and third-party software.
- Must have a valid driver’s license and have daily access to an insured and personal vehicle.
- Ability to work during normal business hours (8am - 5pm) with occasional evening and weekend hours.
- Authorization to work in the US.
- Ability to lift up to 20lbs and to bend, sit and stand for varying periods of time during the workday.
