**Job Summary**:
The Patient Care Coordinator is a healthcare professional who serves as a liaison between patients and healthcare providers, ensuring that patients receive the care they need in a timely and efficient manner.
**What You’ll Do**:
- Coordinates and organizes appointments and documentation to facilitate the smooth running of the healthcare environment.
- Communicates with patients and their families to explain procedures, treatments, and care plans.
- Collaborates with healthcare providers and multidisciplinary teams to ensure patients' medical, physical, and emotional needs are met.
- Assists patients in understanding their health conditions and treatment options.
- Advocates for patients' rights and needs.
- Maintains patient records and ensure confidentiality in accordance with HIPAA regulations.
- Tracks and report on patient care outcomes and identifies opportunities for improvement.
- Assists in the discharge planning process to ensure a smooth transition of care.
- Operates with the highest level of conduct, integrity, and confidentiality.
- Complies with local, state, and federal regulations, company policies and best practices.
- Reports any issues or incidents promptly.
- Participates in quality improvement activities in the healthcare setting.
- Performs other duties as assigned.
**What You’ll Need**:
- Excellent written and verbal communication skills.
- Proficiency in electronic health records and clinical software systems.
- Familiarity with electronic medical record (EMR) systems and other healthcare software.
- Knowledge of HIPAA and other relevant healthcare regulations.
- Outstanding problem-solving and organizational abilities.
- Accuracy and attention to detail.
- Ability to maintain confidentiality of conversations, data records, files, and other vital information.
- Strong interpersonal skills, with the ability to establish rapport and maintain effective relationships with clients.
- Ability to interact effectively with patients, families, and healthcare teams.
- Strong clinical skills and knowledge.
- Commitment to continuous learning and professional development.
**Education and Experience**:
- High school diploma or equivalent required.
- Bachelor's degree in healthcare administration, nursing, or a related field preferred.
- Minimum two years of experience in a healthcare setting, such as a hospital, clinic, or doctor's office.
**Physical Requirements**:
- Prolonged periods sitting, standing, bending, reaching.
- Must be able to lift up to 20 pounds at times.
