They will engage patients and encourage them to take an active role in their health by providing them with the tools necessary to make healthy lifestyle choices, adopt life-long healthy behaviors, close gaps in care, and reduce unnecessary ER utilization and hospital readmissions.Duties and ResponsibilitiesThe duties include, but are not limited to:Implement and support individualized patient care plansAddress patient risk factors and/or obstacles to care including hospital/ER utilizationFacilitate engagement between PCP and patientDemonstrated ability to influence others while motivating them to changeParticipation in required care coordination conferencesMaintain predetermined patient caseloadExercise sound clinical judgment, seek advice and sign off from Registered Nurse as appropriateTake initiative in finding solutions to difficult and/or sensitive problemsParticipate in quality gap closuresWork as patient advocatePerforms miscellaneous job-related duties as assignedQualifications or Education, Training and ExperienceCurrent NJ Practical Nurse LicenseTwo (2)+ years of experience in one or more of the following settings: Acute Care, Ambulatory, Specialty, Physician Office, or Home HealthMust have valid driver license and ability to travelKnowledge and Skills/Expected CompetenciesWillingness & ability to interact with healthcare personnel within a variety of clinical settingsUnderstanding of HIPAA, Confidentiality Obligations and nursing best practiceDemonstrated ability to work as part of a teamExcellent interpersonal and organizational skillsCandidate must be able to execute workflows and work together with RNs and other members of the multi-disciplinary care teamKnowledge of Payor programs and Quality MetricsAbility to review clinical data to address care opportunities and healthcare utilization trendsCritical thinking skills & able to adapt to fast complex working environmentStrong autonomous problem-solving abilityExcellent time management skillsMultitasking and prioritization working in a high-volume environment with minimal supervisionAbility to undertake a variety of diverse care tasks and work in a multidisciplinary teamCompassionate and able to handle stressDocumentation accuracy in medical recordAdaptable to new technologies and softwarePhysical/Mental Demands and Work EnvironmentThe physical demands described are representative of those that must be met by an employee to successfully perform the essential functions of this job.
