Position Overview

The Patient Care Liaison - Licensed Practical Nurse (PCC-LPN) plays a crucial role in the coordination of patient care in partnership with the healthcare team. This position involves the support of tailored care strategies, addressing patient challenges, and enhancing communication between the Primary Care Physician and the patient.

  • Develop and implement personalized patient care strategies
  • Identify and mitigate patient challenges and barriers to care
  • Enhance communication between PCP and patient
  • Engage in care coordination meetings
  • Manage a designated patient caseload
  • Utilize sound clinical judgment in decision-making
  • Contribute to quality improvement initiatives
  • Act as an advocate for patients
  • Perform other job-related responsibilities as needed

Qualifications

- Valid NJ Practical Nurse License
- Minimum of two (2) years of experience in healthcare environments
- Current driver's license with the ability to travel
