The Patient Care Resource Coordinator is responsible for executing administrative tasks related to the progression of care and transitions throughout the healthcare continuum. This role involves collaboration with nurse care coordinators, social workers, physicians, and other members of the care team. The Coordinator acts as a patient and family advocate, aligning with Mass General Brigham's commitment to delivering exceptional care, ensuring high levels of customer satisfaction, and optimizing resource utilization.

Key Responsibilities:

1. Provides comprehensive administrative support to the care team, patients, and their families:

a. Assists with administrative duties and communication regarding post-discharge care (e.g., follow-up appointments, equipment orders, etc.).

b. Facilitates administrative processes for equipment ordering, form completion, and medication authorizations.

c. Distributes essential forms and documents to adhere to regulatory requirements.

d. Coordinates various types of patient transportation under the guidance of the care team.

e. May engage in family meetings and interdisciplinary discussions to provide and gather relevant input.

f. Completes necessary administrative documentation as directed by the care team.

2. Gathers, confirms, and verifies critical patient information (e.g., demographics, healthcare proxy, benefit verification, and pharmacy preferences).

3. Maintains up-to-date knowledge and reference materials on resources available to patients and their families:

a. Serves as a resource for post-acute care options, including insurance requirements and facility information.

b. Identifies and connects patients with community services (e.g., transportation, food assistance, day programs, and financial aid).

c. Communicates and collaborates with various social agencies, clinics, and community organizations under the care team's direction.

Qualifications:

Education:

1. High School Diploma or GED is required.

2. An Associate's Degree or Bachelor's Degree is preferred, along with healthcare experience, particularly in extended care facilities and community organizations.

3. Preferred experience includes hospital discharge planning, long-term care facilities, community health, or utilization review.

Knowledge and Skills:

1. Strong interpersonal skills to effectively engage with diverse staff, patients, families, and community organizations.

2. Extensive understanding of regulations, community organizations, state and federal systems, medical terminology, and healthcare levels.

3. Ability to manage a dynamic workload and adapt to changing priorities while working proactively and independently.

4. Basic typing and computer data entry skills are necessary, with experience in personal computer applications being advantageous.

5. Flexibility to thrive in a constantly evolving environment.
