**Who We Need**
We’re looking for someone with the following skills, experience and credentials:
- Creates and maintains consumer charts including filing, thinning, closing, etc. Completes ROI packets as requested. Assists with the processing of PCP notifications, medical record requests, and refill requests.
- Supports the Morning Meeting and weekly Integrated Care Meeting protocols through management of the meeting logs and daily assignment schedules.
- Manages the team referral status roster, including collection of baseline data.
- Ensures compliance & adequate services, additional job duties may be required to meet the needs of the program and or department.
**Job Requirements**
- Minimum HS diploma/GED and two (2) years of relevant experience in a behavioral health setting.
- Licensures: AZ valid Driver’s License, vehicle with state minimum insurance coverage and able to meet Copa Health company driving requirements.
