GENERAL DESCRIPTION OF DUTIES

The purpose of this position is to support the Agency’s needs by providing administrative oversight and management of Supportive Housing Programs.

ESSENTIAL JOB FUNCTIONS

  • Oversee the day to day management of the Housing Services programs including supervision of program staff.
  • Manage annual program budget.
  • Manage program operations including outreach, intake, data collection, provision of on-site services, program participant grievance and termination process.
  • Assist clients in apartment searches, selections and lease execution.
  • Manage rent payment schedules to landlords.
  • Coordinate communication with landlords and clients regarding apartment repairs and any emergencies related to apartments.
  • Oversee the development of Individual Service Plans for clients. Capture essential customer data and records in the STS Case Management data base.
  • Provide on call coverage for program issues.
  • Provide coordination and oversight of program activities.
  • Provide oversight of all demographic reporting, funder reports and overall program record keeping.
  • Monitor and order bus passes and tokens as needed for clients.
  • Attend client case reviews, presentations etc.
  • Facilitate and coordinate program related activities i.e. team meetings, day to day crisis intervention, staff questions regarding client care.
  • Assign new clients to available case managers.
  • Coordinate and facilitate weekly case management/CAN team meetings.
  • Hire, train, supervise and evaluate Case Managers.
  • Coordinate and conduct in-house client meetings and/or workshops.
  • Meet weekly with staff to provide direction, support, encouragement, advice and address performance issues.
  • Meet with direct supervisor on a weekly basis to communicate any programmatic changes and/or problems.
  • Monitor compliance with length of stay policy tracking client stay, case reviews, notification and termination of clients. Lead case review process.
  • Maintain, monitor, and ensure compliance with STS, HMIS, CAN and HUD Standards
  • Regular attendance
  • Attend and complete all mandatory trainings

ADDITIONAL JOB FUNCTIONS

  • Develop new landlord contacts.
  • Attend outside agency meetings related to the program.
  • Provide program presentations to outside agencies.
  • Perform all other duties as assigned.

MINIMUM TRAINING AND EXPERIENCE

Education: Bachelor’s degree in Human Services required.

Minimum Years of Experience in: one (1) year of experience in Human Service Programs. Minimum of three (3) years’ experience in working with clients with substance abuse, mental illness and chronic homelessness issues required.

Demonstrate skills in: written and verbal communication skills are required. Demonstrates the ability to work in collaboration with a variety of service providers.

Knowledge of: individuals from diverse ethnic and economic backgrounds required.

Active Driver License in good standing required upon hire and throughout employment

Physical and TB Required: YES. A medical and/or physical examination, including a Tuberculin test is required prior to appointment.
