DUTIES:
- Help clinicians and their clients and families identify and access community support and/or a community case manager.
- Assist in all admission paperwork and inform perspective guardians of documentation required for completion of admitting paperwork.
- Assist the clinician in documenting client and family treatment/visit plans in coordination with the client and family and with the oversight of the supervisor.
- Develop resources for clients and families and maintain positive working relationships with resources.
QUALIFICATIONS:
High School Diploma or equivalent required. Bachelor's degree preferred. Five (5) or more years' experience in a related field accepted.
