Overview: Campbell Street Senior Living, a leading provider of long-term care, skilled nursing, assisted living, and independent living facilities across the Midwest, is seeking a dedicated Central Intake Coordinator to join our team. This role is essential in managing and streamlining the intake process for prospective residents, ensuring a smooth transition into our facilities.

Key Duties and Responsibilities:

  • Intake Management: Oversee the central intake process for all inquiries related to long-term care, skilled nursing, assisted living, and independent living. Ensure that all potential residents are provided with accurate information and a seamless entry process.
  • Coordination: Act as the primary point of contact for referrals, families, and potential residents. Coordinate with internal teams to ensure that all required documentation and assessments are completed in a timely manner.
  • Communication: Provide exceptional customer service through effective communication with families, referral sources, and other stakeholders. Address inquiries and concerns promptly and professionally.
  • Documentation: Maintain accurate and confidential records of all intake processes, including assessments, communication logs, and resident information.
  • Collaboration: Work closely with facility administrators and care teams to facilitate smooth admissions and transitions. Ensure that all aspects of the intake process align with facility standards and regulations.
  • Reporting: Prepare and analyze reports related to intake metrics, including conversion rates and response times. Utilize data to identify trends and areas for improvement.
  • Compliance: Ensure adherence to all state and federal regulations related to admissions and resident care.

Qualifications:

  • Education & Experience: Associate’s or Bachelor’s degree in Healthcare Administration, Social Work, or a related field preferred. Prior experience in skilled nursing, senior housing, or a similar healthcare setting is preferred but not required.
  • Skills: Excellent communication, organizational, and interpersonal skills. Ability to manage multiple tasks efficiently while maintaining attention to detail. Proficiency in using CRM systems and office software.
  • Knowledge: Familiarity with long-term care and senior living regulations and best practices is desirable.
