Position Summary

Coordinate and manage the intake process for individuals seeking support services. Facilitate the development of comprehensive service plans tailored to each person's needs. Assist individuals in accessing appropriate resources and crisis intervention. Act as a mediator to resolve conflicts related to service delivery, maintain accurate records, and advocate for the well-being of those we serve.

Essential Job Functions

  • Process service requests by conducting thorough assessments, intakes, social history reviews, and determining eligibility for individuals seeking support.
  • Lead and facilitate interdisciplinary team meetings to develop comprehensive service plans. Utilize effective communication, collaboration, and teamwork skills to ensure a positive and productive planning process.
  • Advocate for and assist individuals in receiving the services identified within their personalized service plan.
  • Maintain regular contact with individuals, service providers, families, guardians, and team members through face-to-face meetings and other communication channels.
  • Empower individuals to achieve self-direction by providing support and guidance that enables them to reach their full potential.

Qualifications

  • A Bachelor's degree from an accredited institution in a field such as behavioral sciences, education, healthcare, human service administration, or the social sciences is required. A minimum of one year of relevant experience in delivering human services to individuals with similar needs is also necessary. Alternatively, an Iowa license to practice as a registered nurse with three years of experience in providing nursing or human services to this population group will be considered.
  • Demonstrate the ability to exercise sound judgment and make independent decisions effectively.
  • Possess the skills and resources to meet agency transportation requirements.
  • Be proficient in using standard office equipment on a daily basis.
  • Exhibit strong communication, interpersonal, teamwork, and organizational skills. Successfully implement agency policies and procedures through verbal, written, and presentation methods.
  • This position requires Medium physical work, including occasional lifting of up to 50 pounds, pushing/pulling up to 60 pounds, frequent standing, walking, reaching, bending, crouching, and stooping. Occasional climbing of stairs without railings may also be necessary.
