Job Title: Housing Case ManagerFull-time: 40 hours/weekPosition Reports To: Programs SupervisorPositions Reporting to the Position: NoneSalary Range: Starting at $50k annually
Overview: The HRSN Housing Case Manager is responsible for providing comprehensive case management services to households experiencing housing crises. The case manager will work with clients to identify and acquire safe and stable housing. This position will adhere to standards set forth by the County and Just Compassion EWC. Essential Duties and Responsibilities:
Housing Search and Placement

  • Provide safe and stable housing solutions for individuals using HRSN funding,
  • Working with clients to develop an individualized and client-driven housing plan that supports their needs and long-term housing goals.
  • Assist participants in identifying suitable housing options.
  • Facilitate housing search and placement process.

Financial Assistance

  • Provide financial assistance for rent, move-in costs, and other housing-related expenses.
  • Ensure proper documentation and compliance with financial management protocols

Case Management and Supportive Services

  • Provide ongoing case management and support services tailored to participants’ needs.
  • Provide clients with necessary life skills to develop/maintain independent living.
  • Coordinate with other service providers to ensure that clients are receiving comprehensive wrap-around services.

Program Compliance and Documentation

  • Ensure participant enrollment in our client database, and track their progress with case notes.
  • Ensure that necessary reporting data is up-to-date and accurate.

Policy and Procedure Adherence

  • Stay informed and comply with Just Compassion and County policies, procedures, and program standards.

Miscellaneous

  • Additional Tasks as assigned by Programs Supervisor and Funding Entities.

Minimum Qualifications:

  • High School Diploma or Equivalent
  • Strong organizational and communication skills
  • Ability to maintain confidentiality.
  • Ability to utilize basic computer software (Office Suite, Google, HMIS, etc).
  • A valid drivers license and car insurance is REQUIRED
  • A reliable vehicle is REQUIRED

Preferred Qualifications:

  • Bachelor’s degree.
  • Experience working with unhoused individuals
  • Bilingual Preferred
  • Case Management
  • Excellent computer skills (Office Suite, Google, HMIS, etc.).

Physical Requirements:

  • Ability to walk up and down stairs.
  • Ability to get up and down off from the floor, unassisted.
  • Ability to stand for a combined total of more than 4 hours a day.
  • Ability to lift up to 20 pounds, unassisted.
  • Ability to assist in physical set-up activities (e.g., setting up and rearranging folding tables and chairs, etc.)
  • Ability to perform activities while twisting and turning.
