Central City Concern (CCC) has been a trusted partner in the Portland community since 1979, dedicated to fostering meaningful, long-term change for individuals experiencing or at risk of homelessness. Each year, we assist over 13,000 individuals through affordable housing, comprehensive health care, addiction recovery services, and employment support.
Role Overview:
As a vital member of a multi-disciplinary team, the RN Case Manager provides intensive case management and integrated dual diagnosis treatment. This role addresses a wide range of treatment needs, including crisis intervention, substance abuse treatment, medication management, and access to health care and housing.
Key Responsibilities:
- Engage in outreach activities to connect with clients in a supportive manner.
- Gather comprehensive client histories through interviews and records review.
- Conduct and document mental health assessments, including clinical formulations.
- Collaborate with clients to develop personalized treatment plans.
- Provide direct nursing care and delegate tasks based on patient needs.
- Assess and triage clients in various settings.
- Participate in clinical meetings to enhance treatment strategies.
- Formulate individualized nursing care plans and monitor client progress.
- Administer medications and treatments as per established protocols.
- Coordinate with housing providers to ensure effective treatment delivery.
- Maintain accurate client records in compliance with program standards.
Qualifications:
- Successful completion of an accredited nursing program (AD, BSN, or AA degree).
- Current RN license in Oregon.
- Certification with MHACBO as a QMHA-R or QMHA I.
- Minimum of two years' experience in psychiatric or addiction nursing.
- Valid driver's license and availability for after-hours coverage.
Preferred Skills:
- Knowledge of mental health treatment methods and crisis intervention.
- Strong communication and problem-solving abilities.
- Ability to work collaboratively in a team-oriented environment.
