Key Responsibilities:

  • Provide evidence-based case management services to residents, focusing on housing retention and stability.
  • Conduct thorough intake and assessments to understand residents' needs and develop personalized plans.
  • Implement Housing First and harm reduction interventions to address residents' challenges.
  • Assist residents in obtaining stable sources of income, promoting financial stability and self-sufficiency.

Requirements:

  • Bachelor's degree in human services or a related field; Master's degree preferred.
  • At least one year of case management experience, with a focus on supportive housing.
  • Three years of experience working with diverse populations, preferably in a supportive housing setting.
  • Experience working with vulnerable adults, including those with complex needs.
  • Proficiency in Microsoft Office, with excellent communication and interpersonal skills.
