Position Summary:
In collaboration with Clackamas Apartments, Just Compassion is seeking a Housing Support Case Coordinator to provide essential case management services. Clackamas Apartments offers 20 units of affordable housing tailored for households facing severe and persistent mental health challenges, who are at or below 50% of the Area Median Income (AMI). The Housing Support Case Coordinator will play a vital role in delivering on-site case management to assist residents in achieving and maintaining housing stability.
Key Responsibilities:Case Management:- Deliver personalized and resident-focused case management services.
- Create and execute tailored service plans that address residents' housing stability, mental health, and overall wellness.
- Conduct regular follow-ups with residents to assess progress and modify service plans as necessary.
Housing Stability Support:
- Provide ongoing assistance to residents to ensure long-term housing stability.
- Connect residents with community resources, including mental health services and employment support.
- Support residents in transitioning from intensive case management and rental assistance when appropriate.
Collaboration and Referrals:
- Work with other service providers to ensure residents receive comprehensive support.
- Facilitate referrals for households already receiving housing assistance who require ongoing case management.
- Engage with the Housing and Community Development Department (HCDD) and other stakeholders for seamless service delivery.
Compliance and Documentation:
- Adhere to the principles and expectations set forth by Clackamas County and relevant funding sources.
- Maintain accurate and current case notes, service plans, and documentation.
- Ensure compliance with funding restrictions and program guidelines.
Additional Duties:
- Perform other tasks as assigned by the supervisor or required by funding entities.
Qualifications:Minimum Requirements:
- Bachelor's degree in Social Work, Psychology, or a related field.
- Strong organizational and communication abilities.
- Commitment to confidentiality.
- Proficiency in basic computer applications (Office Suite, Google, HMIS, etc.).
- A valid driver's license and car insurance are mandatory.
- A reliable vehicle is mandatory.
Preferred Qualifications:
- Master's degree in Social Work, Psychology, or a related field.
- Bilingual candidates are preferred.
- Experience working with individuals experiencing homelessness or those with severe mental health issues.
- Case management experience.
- Advanced computer skills (Office Suite, Google, HMIS, etc.).
Physical Requirements:
- Ability to navigate stairs.
- Ability to get up and down from the floor without assistance.
- Ability to stand for extended periods.
- Ability to lift up to 20 pounds independently.
- Ability to assist with physical setup tasks (e.g., arranging tables and chairs).
- Ability to perform tasks involving twisting and turning.
