Position Title: Housing Support Case Manager

Employment Type: Full-time, 40 hours per week
Reports To: Programs Supervisor
Salary: Starting at $50,000 annually

Role Summary:

The Housing Support Case Manager plays a crucial role in delivering extensive case management services to families and individuals facing housing instability. This position involves collaborating with clients to secure safe and stable housing solutions while adhering to the guidelines established by Yamhill County and Just Compassion EWC.

Key Responsibilities:Housing Search and Placement
  • Facilitate access to safe and stable housing options utilizing HRSN funding.
  • Collaborate with clients to create personalized housing plans that align with their unique needs and long-term goals.
  • Assist clients in identifying appropriate housing alternatives.
  • Guide clients through the housing search and placement process.
Financial Support
  • Provide financial assistance for rent, move-in expenses, and other housing-related costs.
  • Ensure compliance with financial management protocols and maintain accurate documentation.
Case Management and Support Services
  • Deliver ongoing case management and tailored support services to meet the needs of participants.
  • Equip clients with essential life skills for independent living.
  • Coordinate with various service providers to ensure comprehensive support for clients.
Program Compliance and Documentation
  • Maintain participant records in the client database and document progress through case notes.
  • Ensure accuracy and timeliness of necessary reporting data.
Policy Adherence
  • Stay informed and comply with the policies and procedures of Just Compassion and County standards.
Additional Responsibilities
  • Perform other tasks as assigned by the Programs Supervisor and funding entities.
Qualifications:Minimum Requirements:
  • High School Diploma or equivalent.
  • Strong organizational and communication abilities.
  • Ability to maintain confidentiality.
  • Proficiency in basic computer applications (Office Suite, Google, HMIS, etc.).
  • A valid driver's license and car insurance are required.
  • A reliable vehicle is mandatory.
Preferred Qualifications:
  • Bachelor's degree.
  • Experience working with individuals experiencing homelessness.
  • Bilingual candidates are preferred.
  • Experience in case management.
  • Excellent computer skills (Office Suite, Google, HMIS, etc.).
Physical Requirements:
  • Ability to navigate stairs.
  • Ability to get up and down from the floor without assistance.
  • Ability to stand for extended periods.
  • Ability to lift up to 20 pounds unassisted.
  • Ability to assist with physical setup activities.
  • Ability to perform tasks involving twisting and turning.
