Supportive Housing Case Manager

Overview: Just Compassion is partnering with Clackamas Apartments to provide supportive housing case management. Clackamas Apartments provides 20 units of affordable housing specifically designed to support households with severe and persistent mental illness who are at or below 50% of the Area Median Income (AMI). The Supportive Housing Case Manager will be a crucial part of the program, delivering site-based case management services to help residents maintain housing stability. The role is focused on assisting households that have secured permanent housing and have expressed a need or desire for supportive services.Essential Duties and Responsibilities:
Case Management Services:
- Provide individualized and tenant-driven case management services to residents.
- Develop and implement personalized service plans that address residents' needs and goals related to housing stability, mental health, and overall well-being.
- Conduct regular check-ins with residents to monitor progress and adjust service plans as needed.
Housing Stability:
- Assist residents in maintaining long-term housing stability by providing continuous support and resources.
- Facilitate connections to community resources, such as mental health services, employment assistance, and other supportive services.
- Help residents transition away from intensive case management and/or rental subsidies when appropriate, supporting their "graduation" from the program.
Collaboration and Referral:
- Coordinate with other service providers and programs to ensure residents receive comprehensive support.
- Refer households to this program if they are already receiving housing support through alternative programs but require continuous case management services.
- Work closely with the Housing and Community Development Department (HCDD) and other stakeholders to ensure seamless service delivery.
Program Compliance and Documentation:
- Adhere to all guiding principles and expectations outlined by Clackamas County and relevant funding sources.
- Maintain accurate and up-to-date case notes, service plans, and other required documentation.
- Ensure compliance with funding restrictions and program requirements.
Miscellaneous:
- Additional tasks as assigned by supervisor or implemented by funding entities.
Minimum Qualifications:
- Bachelor’s degree in Social Work, Psychology, or related field.
- Strong organizational and communication skills
- Ability to maintain confidentiality.
- Ability to utilize basic computer software (Office Suite, Google, HMIS, etc).
- A valid drivers license and car insurance is REQUIRED
- A reliable vehicle is REQUIRED
Preferred Qualifications:
- Master’s Degree in Social Work, Psychology, or related field
- Bilingual Preferred
- Experience working with unhoused individuals and/or individuals with severe and persistent mental illness.
- Case Management
- Excellent computer skills (Office Suite, Google, HMIS, etc.).
Physical Requirements:
- Ability to walk up and down stairs.
- Ability to get up and down off from the floor, unassisted.
- Ability to stand for a combined total of more than 4 hours a day.
- Ability to lift up to 20 pounds, unassisted.
- Ability to assist in physical set-up activities (e.g., setting up and rearranging folding tables and chairs, etc.)
- Ability to perform activities while twisting and turning.
