Position Summary:


The Case Manager will be responsible for providing case management services to individuals in the Multnomah County Deflection Center. . This position will work in collaboration with a multidisciplinary team to assess the needs of the individuals participating in deflection services. . The goals of the case manager within the deflection center will be to offer a plan for individuals to access needed medical, social, educational, substance use, mental health, and other applicable services. Contacts and interactions vary and may involve multiple constituencies such as direct interaction with law enforcement, emergency responders, , community-based organizations, Multnomah County staff, and the general public, Duties and responsibilities:


Work with clinical staff to identify individuals to participate in services.

Conduct evaluations to determine the needs of individuals such as basic,, educational, medical, or housing.

Assist individuals in applying for necessary public assistance and Oregon Health Plan enrollment or re-enrollment.

Develop a list of community resources and agencies and provide needed services to individuals.

Provide a comprehensive, individualized care plan that addressed all identified needs to include but not limited to medical care, prevention outreach, treatment readiness, medications, housing, financial assistance, public entitlement applications, vocational/employment services, legal issues, substance abuse and mental health treatment.

Work with medical and support staff to gather screening information,and to coordinate continuity plans.

Coordinate services required to implement the service care plan.

Make referrals to address individual’s social service/support service needs.

Provide individuals monitoring to assess the efficacy of the service care plan.

Participate in meetings and training, as required.

Collect and submit required data for monthly reports.

Perform other duties within the scope and purpose of the job.


Knowledge, Skills & Abilities, Education and/or Experience:


● Bachelor’s degree in behavioral sciences, or work experience equivalent to a bachelor’s degree

● knowledge of:

● mental health substance use/missuse/disorder

● Substance use disorders and their treatment by a variety of appropriate therapeutic interventions including psychopharmacology.

● Physical diseases and their treatment

● Community resources and linkage to those services

● Computer literate

● Ability to craft professional emails

● Active listening skills and attention to details

● Expert documentation of medical records

● Basic data and analytical abilities

● Organizational skills

● Typing and computer skills

● Time-management abilities

● Attention to detail

● Responsibility and professionalism.

● Good communication skillsComply with HIPPA/State compliance regulations

● Ensure staff compliance with EMR and uphold the 48-hour documentation standard

● All other duties as assigned



● Ability to learn in-house software.


● Sobriety: If recovering, two (2) years in recovery.


● Other: The incumbent must continually demonstrate excellent oral and written communication skills. This position may include weekend and/or evening duties as needed or required.
