CATCH is a non-profit housing developer, with affordable housing residential buildings located throughout New York City. We seek a full-time, on-site Social Service Coordinator/Community Organizer to join the management team at Logan Gardens in Upper Manhattan, one of our residences. The ideal candidate has education and/or experience in social work or human services. More specifically, the Social Service Coordinator will link older adults and people with disabilities to supportive services and other community resources and will spark a sense of community in the building.

The Role:
Help residents age in place and remain living independently in their own homes
Advocate for adequate, timely and cost-effective provision of services
Assess service needs of residents and link them to the appropriate providers and community resources
Assist residents with applications for benefits and entitlement programs
Facilitate programs and services such as health and wellness, or transportation
Develop a resource directory of local social service agencies and providers
HUD reporting/compliance
Other related duties as assigned

Youre a great candidate if you:
Are fluent in English and Spanish (strongly preferred)
Have 1-3 years of related experience
Enjoy working with elderly and disabled populations
