Basic Function:

Quality Insights End Stage Renal Disease Networks are federally funded programs responsible for quality improvement and data collection activities for Medicare’s End Stage Renal Disease (ESRD) Program. The ESRD Network’s mission is to lead improvement in the delivery of quality of care to people with chronic kidney disease, valuing each member of the renal community and his or her role in achieving this goal. The Patient Engagement Specialist assists the ESRD project team and processes patient grievances and mediates involuntary discharges and transfers.

Organizational Relationships:

Reports to the Project Director

Role and Responsibilities:
  • Serve as a member of the ESRD Patient Services team.
  • Works with staff to facilitate patient and family engagement as outlined in the statement of work in partnership with the project team.
  • Processes patient grievances and provides back-up to other patient services project team members
  • Mediates IVDs/IVTs
  • Maintains computerized log of patient and provider contacts in CMS-directed utility
  • Provides technical assistance and develops and conducts community outreach activities for patients and providers
  • Assists in patient/family engagement activities, such as coordination of Patient Advisory Committee, and patient subject matter experts, facility patient liaisons
  • Assists in the design and implementation of Quality Improvement Activities and education materials
  • Assists in the development and coordination of the emergency response plan
  • Other duties as assigned
Supervisory Responsibilities:

None

Skills / Professional Competencies Required:
  • Excellent interpersonal skills and the ability to interact professionally with a varied population (patients, nurses, doctors, government officials, etc.)
  • Ability to address patient and provider concerns in a timely manner and with tact, empathy and a focused approach to conflict resolution
  • Superb written and oral communication skills
  • Demonstrated problem-solving abilities
  • Strong organizational skills
  • Proficiency in MS Office suite
Minimum Education/Training Required:

Master’s degree in social work or similar accredited counseling credentials

Minimum Experience Required:

One to three (1-3) years of professional experience as an ombudsman/advocate for patients, preferably those with a chronic or terminal condition, with emphases on conflict management/resolution, community outreach and training. Direct experience with ESRD patients strongly preferred.

Licensure/Certification Requirements:

Licensed Social Worker (LSW) strongly preferred
