This position will be responsible for partnering with the HIV Prevention Coordinator in participating and/or leading key programmatic activities such as implementing program strategies, evaluating program outcomes, and ensuring full integration of HIV Prevention Program activities at CRCH. The HIV program is in the beginning stages of development and CRCH is seeking individuals motivated to assist in building this program from the ground up. This role would also include coordinating sexually transmitted infection (STI) workflows and implemetation as well as providing clinical education to patients and triaging/ following up on health related concerns. This role will assist with developing relationships with community partners in order to better identify outreach and education strategies in surrounding community. This position will work with other CRCH teams for outreach campaigns relating to HIV /STI prevention. Additionally, the STI/HIV Clinical Coordinator will work with the various clinical staff members to ensure coordination of care.

YOUR RESPONSIBILITIES

These responsibilities include but are not limited to:

• Communicate with medical team to see if efforts to increase HIV/STI screenings rates set forth are working and consult with HIV Prevention Coordinator when needed

• ln conjunction with HIV Prevention Coordinator, link patients with HIV to care within a timely manner by solidifying relationship with providers and the care teams.

• Work closely with HIV Provider Champion as it relates to patient care management and coordination which includes helping to advise HIV Provider Champion in PreP and PEP medication initiation and management.

• Assist HIV Prevention Coordinator with researching, creating, and updating relevant policies and procedures, and assure they are implemented across the organization effectively.

• Along with the HIV Prevention Coordinator, attend initial and continuing education training programs, TA meetings, and EHE Steering Committee meetings. Inform new evidence- based practice to teammates as needed.

• Collaborate with patients and interdisciplinary teams to develop goals for patient care, while providing patients with the necessary education needed for STI/HIV prevention or disease management.

• Assist the HIV Prevention Coordinator with scheduling and coordinating training for providers and all other staff on HIV program.

• Assist the HIV Prevention Coordinator with developing a streamlined HIV Confidential Case Reporting protocol for the organization.

• Develop streamlined STI workflows and reporting protocols for the organization, communicate new workflows to CRCH employees.

• Coordinate regular meetings with key clinical staff to discuss infectious disease updates including updates in workflows, evidenced based practice and new workflows as well as integrate into care team meetings; communicate any findings between departments and support improvement initiatives.

• Actively support providers and clinical staff in navigating newly diagnosed patients with HIV, or those that are new to case with existing diagnosis.

• Coordinates related HIV program PDSA cycles across all service lines within the organization.

• Organize and maintain appropriate educational materials for patients and staff in regard to targeted efforts.

• Works to develop a team-based model approach for planning and executing initiatives.

• Work with other members of population health team as needed to meet overall team goals.

YOUR QUALIFICATIONS, COMPETENCIES, TRAITS

• Associates or Bachelor’s degree in nursing required. Active MA RN License

• 3+ years of experience in public health or community health preferred

• Project management experience a plus but not required

• Experience in infectious disease preferred with a focus in STI/HIV care

• Microsoft Office experience required

• Comfortability and understanding of program development and willingness to demonstrate the ability to overcome barriers while remaining innovative.

• Experience in communicating well within an interdisciplinary team.

• Bilingual (Spanish and/or Portuguese) is a plus but not required
