Job Summary:

The Staffing and Patient Care Coordinator is expected to perform a variety of duties in the coordination of scheduling services of clients. This position is responsible for the day-to-day operations in the staffing department, which includes scheduling patients with Home Health Aides to provide the highest quality of service.

Job Responsibilities:

  • Maintain Electronic Schedule in ContinuLink Database
  • Assign Home Health Aids and Personal Care Assistants to Clients
  • Monitor and ensure utilization hours for patients
  • Manage day-to-day operations of our scheduling department by assigning work for the schedulers
  • Oversee the on-call schedule and participate in the on-call rotation
  • Reflect the values of Vertical Home Health, while following staffing and company policies
  • Provide consistently professional interaction with Peers
  • Field new patient knowledgeably inquires over the phone for CICOA and other referral sources
  • Coordinate home health aides' schedules daily
  • Follow up on home health assignments and patient services
  • Coordinate and maintain monthly patient schedules in the system
  • Enter and maintain accurate records for the patient and home health aides in system

 

Minimum Requirements:

  • Experience with scheduling in a home healthcare environment.
  • Minimum 6 months of experience with scheduling in a home healthcare environment.
  • High School Diploma or equivalent
  • Ability to effectively prioritize tasks in a fast-paced environment
  • Intermediate proficiency with Microsoft Office, including Excel
  • Excellent spoken and written English communication skills
  • Professional and Friendly Demeanor, in-person and on phone
  • Have availability to be on-call

****Pay based on experience****
