Compensation Range Starting at $22.50 per hour based upon experience

Compensation will be dependent upon geographic region, education, and experience

Job Description

The Care Coordinator coordinates transition of client care between inpatient setting and outpatient setting. Including: Sober Living, Intensive Outpatient Programs, Therapists, Psychiatrists, and/or other providers as clinically indicated. Meets with clients on a regular basis to assess aftercare needs and works closely with treatment team and clinical outreach to provide appropriate referrals. This is maintained through coordination with client’s assigned therapist, assigned admissions coordinator, clinical outreach or other appropriate staff to ensure quality and appropriate care. This is conducted through weekly face-to-face 30-minute sessions with clients throughout their residential stay. More meetings may be required for longer stays or extenuating circumstances with client. 

Responsibilities

  • Demonstrates the ability to develop a thorough continuing care plan that is in alignment with treatment team recommendations, business development and client’s individual needs
  • Communicates with outside therapists and other appropriate parties as indicated within the compliance of HIPPA requirements
  • Addresses discharge planning needs and concerns from clients. 
  • Initiates Client Discharge Plan within 24 hours of admission, completes and reviews with the client prior to discharge.
  • Responsible for the Coordination of care within 72 hours of admission and Family Contact within 48 hours of admission and documenting both in client’s chart.
  • Facilitate 1-2 aftercare groups as needed per week.
  • Meets weekly or as needed with client
  • Attends weekly Treatment Team meetings to gain needed information from therapists and to communicate important information about each client’s continuing care plan
  • Completes Discharge Planning Progress Notes for each client
  • Documents outbound referrals in the client’s chart and salesforce
  • Maintain knowledge of client discharge date through assigned therapist and admissions team (as dates may change in accordance with insurance)
  • Must meet with clients a minimum of 3 sessions
  • Maintain timely scheduled initial sessions with clients within 6 days of intake.
  • Must meet with client 1 week before discharge to secure Discharge Referrals and suggest possible extension if client’s aftercare plan is not a suitable structure for success.
  • Must meet with clients 1 week before discharge to secure appointments for clients with outside providers. (Psychiatrist and Therapist appointments for clients before discharge, securing bed in Sober Living, etc.)
  • Thorough research of recommended Outpatient services Sober Livings, therapists, psychiatrists) to ensure quality referrals.  Research conducted will include primarily clinical outreach recommended referrals, internet screening for well-trusted services, phone calls to verify appropriateness and reserving available space if transitioning to a sober living.
  • Building effective and lasting relationships with referents and community providers

This is a full-time position that will be Tuesday-Saturday.

Qualifications

  • Required: Minimum of 3 years’ experience in a mental health, chemical dependency setting or healthcare.
  • Required: Experience working with clients and documentation in a hospital and/or mental health facility, Case Management experience required.
  • Preferred:
    • A bachelor's degree in psychology, Social Work or other related field
    • CADC (Certified Alcohol and Drug Counselor) Certificate.
