The Role

Community Care Coordinators at The Council on Recovery will utilize the Pathways Community Hub model to improve client and community outcomes and reduce disparities by coordinating holistic care for high-risk populations. This role will focus on individuals who have mental health and/or substance use disorders, pregnant and parenting women who may be high risk for maternal mortality and severe morbidity. Community Care Coordinators are responsible for community outreach, client engagement and appropriate referrals to community resources. Community outreach consists of recruiting clients in nontraditional settings during nontraditional hours. Locations may not be limited to just residential, shelters, schools, clinics, or other community sites. These efforts may be conducted individually, in partnership with other provider organizations, and/or with other Council teams.


Great Community Care Coordinators will be knowledgeable of client/community resources and have experience developing relationships. They will be flexible and possess excellent organizational skills. Requirements:

EDUCATION AND REQUIREMENTS

· Community Health Worker certification required

· High School Diploma or Equivalency required; Bachelor’s Degree preferred

· Bilingual – fluent English/Spanish (strongly preferred)

· Lived experience with substance use (valued)

· Experience working with community agencies: Healthcare System, Department of Family and Protective Services, Criminal Justice, Substance Use, Mental Health

· Experience conducting assessments, case management and crisis intervention

· Experience with and clear understanding of the needs of individuals with mental health and/or substance use disorders

· Experience with and clear understanding of the needs of women at risk for becoming pregnant, pregnant, and postpartum women

· Experience working with a diverse population

· Valid driver’s license, automobile insurance and reliable transportation
