Position Overview

Patient Care Coordinator / RN Case Manager - Focused on discharge planning (ICU)

We are in search of a dedicated Patient Care Coordinator/Registered Nurse Case Manager who thrives in a dynamic environment.

This role involves assessing, planning, implementing, monitoring, and evaluating the necessary options and services to address an individual's health requirements.

The Patient Care Coordinator will deliver comprehensive ongoing case management services to patients by orchestrating and overseeing their care to fulfill various service needs throughout the continuum of care.

Your efforts will ensure optimal patient outcomes that prioritize quality, service, customer satisfaction, and cost efficiency.

In collaboration with the physician, the Patient Care Coordinator will establish care plans and allocate resources based on the patient's risk assessment.

Additionally, you will support patients and their families in managing illness by enhancing their self-care capabilities and advocating for their consumer rights.

Key Responsibilities
  • Conduct a thorough, goal-oriented, and personalized assessment for patients referred for evaluation, addressing biophysical, psychosocial, environmental, discharge planning needs, and financial status.
  • Facilitate and/or provide community education and outreach for patients as necessary, focusing on health promotion for chronic disease management and wellness information.
  • Ensure the quality and cost-effective utilization of resources while facilitating access to care.
  • Collaborate with community health education and outreach organizations to promote health for children with complex healthcare needs.
  • Prepare and monitor outcome data to identify opportunities for improvement.
  • Compliance with COVID-19 vaccination requirements is mandatory, unless a medical or religious exemption is approved.
  • A Bachelor's degree in nursing or an Associate's degree/technical diploma in nursing with acceptance or current enrollment in a BSN program is required, along with a commitment to advancing education toward an MSN.
  • Experience in ICU settings is preferred.
  • A valid RN license from the Texas Board of Nursing is required.
  • Certification in Care Coordination and Transition Management (CCCTM) by the Medical-Surgical Nursing Certification Board is preferred.
  • A minimum of 3 years of nursing experience in an acute care setting is required.
  • Experience in community health, complex pediatric patient care, home care, case management, managed care, or utilization review is preferred.
