Key Responsibilities
The RN Continuing Care Coordinator plays a vital role in enhancing patient care through collaboration with healthcare teams. This position involves:
- Chronic Disease Management: Developing tailored care plans based on comprehensive nursing assessments, including patient and caregiver education and coordination with interdisciplinary teams.
- Referral Management: Overseeing patient referrals in accordance with established program criteria.
- Performance Data Analysis: Engaging in the creation of workflows and conducting audits to improve care processes.
- Implementation of Evidence-Based Guidelines: Facilitating the integration of evidence-based metrics into care plans, ensuring smooth transitions between various care settings.
Essential Job Functions
Assessment:
- Support patients and families in self-management strategies.
- Identify and address variances in care, adjusting plans as necessary.
Leadership:
- Lead interdisciplinary teams in managing high-risk patients, fostering collaboration and communication.
- Design data collection strategies to evaluate project success and support practitioners in data interpretation.
Critical Thinking:
- Assist patients in navigating the healthcare system to ensure timely access to services.
- Monitor patient compliance with appointments and coordinate necessary scheduling.
Relationship Management:
- Enhance care coordination across the continuum, maintaining positive relationships with healthcare teams.
- Assume accountability for patient loads while supporting colleagues as needed.
This role may involve meeting patients and families in various settings, requiring the ability to assess safety and escalate concerns when necessary.
Qualifications
Education and Experience:
- Associate degree in Nursing required.
- Two years of relevant experience or an advanced degree preferred.
Preferred: Bachelor of Science in Nursing (BSN)
Licensure and Certifications:
- Current Texas RN license required.
Knowledge, Skills, and Abilities:
- Ability to manage interdisciplinary partnerships effectively.
- Experience in implementing program goals to enhance performance.
- Strong organizational and interpersonal skills.
- Proficient in communication and technology use.
