Key Responsibilities

The RN Continuing Care Coordinator plays a vital role in enhancing patient care through collaboration with healthcare teams. This position involves:

  • Chronic Disease Management: Developing tailored care plans based on comprehensive nursing assessments, including patient and caregiver education and coordination with interdisciplinary teams.
  • Referral Management: Overseeing patient referrals in accordance with established program criteria.

Relationship Management:

This role may involve meeting patients and families in various settings, requiring the ability to assess safety and escalate concerns when necessary.

Qualifications

Education and Experience:

  • Associate degree in Nursing required.
  • Two years of relevant experience or an advanced degree preferred.

Preferred: Bachelor of Science in Nursing (BSN)

Licensure and Certifications:

  • Current Texas RN license required.

Knowledge, Skills, and Abilities:

  • Ability to manage interdisciplinary partnerships effectively.
  • Experience in implementing program goals to enhance performance.
  • Strong organizational and interpersonal skills.
  • Proficient in communication and technology use.
