Position Overview
We are seeking a Care Coordinator to play a vital role in transforming the delivery of primary care and enhancing patient experiences.

As a national leader in healthcare, we collaborate with some of the finest primary care physicians, equipping them with cutting-edge digital tools, proven strategies, and ongoing support to inspire better practices and consistent results.

Your Impact
As a key member of our Care Management team, the Care Coordinator will:

  • Engage with high-risk patients, ensuring they have follow-up appointments with their Primary Care Provider (PCP) and that all care gaps are addressed.
  • Collaborate with care team members and external partners to facilitate patient care, including the provision of durable medical equipment and home health services.
  • Request and manage medical records from various healthcare providers to ensure continuity of care.
  • Screen and escalate patient needs to nursing and social work teams as necessary.
  • Support nursing and social work team members in patient follow-up, providing necessary tools and resources.
  • Monitor stable patient populations under the guidance of nursing and social work professionals.
  • Work with patients to address open care gaps, ensuring proper documentation is maintained.
  • Coordinate with local community resources to meet both physical and psychosocial patient needs.
  • Collaborate with multidisciplinary team members to deliver exceptional patient care and improve health outcomes.
  • Participate in special initiatives as directed by market leaders.

Qualifications for Success
To excel in this role, candidates should possess:

  • 3+ years of experience in a medical office or healthcare environment.
  • 2+ years of experience as a Licensed Practical Nurse (LPN), Medical Assistant (MA), Pharmacy Technician, or Community Health Worker preferred.
  • Proficiency with technology, including the Microsoft Office suite.
  • Experience with electronic health records, including data capture and reporting.
