GENERAL SUMMARY

The Resident Service Coordinator (RSC) plays a key role in the Supportive Services Department by providing assistance to residents to connect them to supportive services, health, wellness, and medical services in the community, by public agencies, and/or private practitioners. The RSC helps to promote wellness services and supportive service programs offered by Bethany Community Services both internally and externally through marketing and promotion of programs and services.


ESSENTIAL JOB FUNCTIONS

  • Provides general case management (including intake) and referral services to all residents needing such assistance. *
  • May provide formal case management (i.e. evaluation of health, psychological and social needs, development of an individually tailored case plan for services and periodic reassessment of the resident’s situation and needs) for a resident when such service is not available through the general community.
  • Conduct initial assessments with new residents, identify needs, interview residents and families, follow organizational procedures to collect data, maintain privacy and confidentiality, and provide education, service and support to all residents. *
  • Assess service needs and determine eligibility for public services for elderly residents (ie. MassHealth, Group Adult Foster Care, Senior Care Options, Veterans Benefits, SNAP/EBT, etc.). *
  • Educates residents on service availability, application procedures, client rights, etc., providing advocacy as appropriate *
  • Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual.
  • Works closely with Supportive Services Staff (Wellness Nurses and Health Aides), to develop service plans and regularly communicate status updates of residents receiving services
  • Coordinates care with the resident, family member(s), physician(s), insurance companies, Area Agency on Aging (AAA), Aging Service Access Point (ASAP) and other health care providers and relevant parties.
  • Establishes linkages with all agencies and service providers in the community, creates strong relationships with these agencies, including but not limited to; skilled nursing care centers, local hospitals, rehabilitation centers, home health agencies, home care agencies, hospital discharge planners, and hospice providers.
  • Markets Bethany’s Supportive Service programs, including Assisted Living, to those agencies and providers listed above. Develops referrals from these providers and maintains regular contact as a liaison with key contacts. *
  • Participation as a member of the Bethany Professional Assessment Committee (PAC) which reviews/approves residents for program participation
  • Helps residents build informal support networks with other residents, family and friends.
  • Track resident information and data to provide reports according to established metrics (ie. Number of residents served, type of service provided, etc.)
  • Prepare monthly and annual reports to be published in the Bethany Annual Report.
  • Prepare and file the bi-annual Service Coordinator report with HUD as required
  • Comply with all Bethany Community Services policies and maintains a safe and clean work environment.


OTHER DUTIES AND RESPONSIBILITIES

  • Mandatory Resident Service Coordinator training as outlined in HUD regulations.
  • Attend training and professional development courses and sessions in order to remain current with the industry, health care programs and wellness initiatives.
  • Other duties as assigned as may be required to meet the operational needs of the Supportive Services Department.
  • Local/regional travel for community outreach and marketing, establishing relationships with potential referral sources.

Requirements:

COMPETENCIES

  • Knowledge of Federal and State programs (Medicare, Medicaid, MassHealth, Group Adult Foster Care, Senior Care Options (SCO’s), Home and Community Based Waiver Program, Massachusetts State Home Care Program)
  • Organizational skills, interpersonal skills, excellent verbal and written communication skills
  • Desire to work with an elderly population
  • Intermediate to advanced computer skills are expected. Experience with Word, Excel and the internet is required. Ability to enter data into a Resident Service software will be expected. Training will be provided.


PREPARATION, KNOWLEDGE, SKILLS & ABILITIES

  • A Bachelor’s Degree in Social Work, Gerontology, Psychology, or Counseling is preferred.
  • Applicants without a degree as listed above must have appropriate work experience in resident service coordination or in a related field.
  • Supervisory or management experience is desirable.
  • Training in the aging process, elder services, disability services, eligibility for and procedures of Federal and applicable State entitlement programs, legal liability issues relating to providing service coordination, drug and alcohol use and abuse, and mental health issues.
  • Two to three years’ experience in social service delivery with the elderly and nonelderly disabled.
  • Demonstrated working knowledge of supportive services and other resources for senior citizens and non-elderly disabled.
  • Demonstrated ability to advocate, organize, problem solve and provide results for the elderly and non-elderly disabled.


WORKING CONDITIONS/PHYSICAL DEMANDS

  • Ability to read, write and communicate in English is required.
  • Transportation availability and ability to travel between locations for meetings is required.
