POSITION SUMMARY

The Community Support Services Coordinator (CSSC) is an integral part of the Supportive Services Division, dedicated to assisting residents in accessing essential health, wellness, and medical resources within the community. This role is pivotal in promoting the wellness initiatives and supportive service offerings provided by Bethany Community Services, ensuring effective outreach and engagement with both internal and external stakeholders.

KEY RESPONSIBILITIES

  • Facilitates comprehensive case management and referral services for residents requiring assistance.
  • Conducts formal case evaluations to assess health, psychological, and social needs, crafting personalized service plans when community resources are insufficient.
  • Performs initial assessments for new residents, identifying their needs through interviews and data collection while upholding privacy and confidentiality standards.
  • Evaluates service requirements and determines eligibility for public assistance programs tailored for senior residents.
  • Educates residents on available services, application processes, and client rights, advocating on their behalf as necessary.
  • Monitors the delivery of services from community partners, ensuring consistent communication regarding residents' progress.
  • Collaborates closely with Supportive Services Staff, including Wellness Nurses and Health Aides, to develop and update service plans.
  • Coordinates care among residents, family members, healthcare providers, and relevant agencies to ensure comprehensive support.
  • Establishes and nurtures relationships with community agencies and service providers, including skilled nursing facilities, hospitals, and home care organizations.
  • Promotes Bethany's Supportive Service programs to potential referral sources, maintaining regular communication to foster partnerships.
  • Participates in the Bethany Professional Assessment Committee, contributing to the evaluation and approval of resident program participation.
  • Encourages residents to build informal support networks with peers, family, and friends.
  • Tracks resident data to generate reports reflecting service metrics, such as the number of residents served and types of services provided.
  • Prepares monthly and annual reports for inclusion in the Bethany Annual Report.
  • Files bi-annual Service Coordinator reports as mandated by regulatory requirements.
  • Adheres to all organizational policies and maintains a safe and organized work environment.

ADDITIONAL DUTIES

  • Completes mandatory training as outlined in regulatory guidelines.
  • Engages in ongoing professional development to stay informed about industry trends and wellness initiatives.
  • Performs other duties as assigned to meet the operational needs of the Supportive Services Division.
  • Travels locally for community outreach and relationship-building with potential referral sources.

  • Knowledge of Federal and State assistance programs is essential.
  • Strong organizational, interpersonal, and communication skills are required.

EDUCATION AND EXPERIENCE

  • A Bachelor's Degree in Social Work, Gerontology, Psychology, or Counseling is preferred.
  • Relevant work experience in resident service coordination may substitute for formal education.
  • Experience in supervisory or management roles is advantageous.
  • Training in elder services and related fields is beneficial.
  • Two to three years of experience in social service delivery for seniors and disabled individuals is preferred.

WORK ENVIRONMENT

  • Fluency in English is required for effective communication.
  • Availability of transportation for travel between locations is necessary.
