POSITION SUMMARY

The Community Support Services Coordinator is integral to the Supportive Services Department, facilitating connections for residents to essential health, wellness, and medical resources available in the community, whether through public agencies or private practitioners. This role emphasizes the promotion of wellness initiatives and supportive service offerings provided by Bethany Community Services, both internally and externally, through effective marketing and outreach strategies.

KEY RESPONSIBILITIES

  • Deliver comprehensive case management and referral services to residents requiring assistance.
  • Conduct formal case evaluations to assess health, psychological, and social needs, creating tailored service plans for residents when community resources are insufficient.
  • Perform initial assessments for new residents, identifying their needs, interviewing them and their families, and adhering to organizational protocols to gather data while ensuring privacy and confidentiality.
  • Evaluate service requirements and ascertain eligibility for public assistance programs available to elderly residents.
  • Provide education to residents regarding service availability, application processes, and client rights, advocating as necessary.
  • Oversee the continuous delivery of services from community agencies, maintaining communication with case management and provider agencies regarding residents' progress.
  • Collaborate closely with Supportive Services Staff, including Wellness Nurses and Health Aides, to develop and update service plans for residents receiving assistance.
  • Coordinate care among residents, family members, healthcare providers, and relevant agencies to ensure comprehensive support.
  • Establish and nurture relationships with community agencies and service providers, including skilled nursing facilities, hospitals, rehabilitation centers, and home health agencies.
  • Promote Bethany's Supportive Service programs to relevant agencies, generating referrals and maintaining ongoing communication with key contacts.
  • Participate in the Bethany Professional Assessment Committee, which evaluates residents for program eligibility.
  • Assist residents in building informal support networks with peers, family, and friends.
  • Maintain accurate records of resident information and service data to generate reports based on established metrics.
  • Prepare monthly and annual reports for inclusion in the Bethany Annual Report.
  • Compile and submit the bi-annual Service Coordinator report to HUD as mandated.
  • Adhere to all policies of Bethany Community Services, ensuring a safe and organized work environment.

ADDITIONAL DUTIES

  • Complete mandatory training for Resident Service Coordinators as outlined by HUD regulations.
  • Engage in ongoing training and professional development to stay informed about industry standards, healthcare programs, and wellness initiatives.
  • Perform other duties as assigned to meet the operational needs of the Supportive Services Department.
  • Travel locally and regionally for community outreach and relationship-building with potential referral sources.

QUALIFICATIONS

  • Knowledge of Federal and State programs such as Medicare, Medicaid, and MassHealth.
  • Strong organizational, interpersonal, and communication skills, both verbal and written.

  • Proficiency in computer applications, including Word and Excel, with the ability to enter data into Resident Service software.

EDUCATION AND EXPERIENCE

  • A Bachelor's Degree in Social Work, Gerontology, Psychology, or Counseling is preferred.
  • Relevant work experience in resident service coordination or a related field may substitute for educational requirements.
  • Supervisory or management experience is advantageous.
  • Training in elder services, disability services, and Federal and State entitlement programs is beneficial.
  • Two to three years of experience in social service delivery to elderly and disabled populations is preferred.

WORKING CONDITIONS

  • Fluency in English, both written and spoken, is required.
  • Availability of transportation and the ability to travel between locations for meetings is necessary.
