MDS Nurse Coordinator

Position Overview:
The MDS Coordinator (LPN/LVN), under the supervision of the Director of Nursing (DON), is tasked with overseeing the Resident Assessment Instrument (RAI) process. This includes ensuring the completion of a precise Minimum Data Set (MDS) and gathering data to formulate the Interdisciplinary Plan of Care.
Key Responsibilities:
- Overseeing the Resident Assessment Instrument (RAI) process
- Ensuring the accurate completion of the Minimum Data Set (MDS) and the development of the interdisciplinary Plan of Care
- Participating in the daily Medicare Prospective Payment System (PPS)
- Engaging in Interdisciplinary Team Meetings (ITM)
- Fostering and maintaining constructive relationships with all healthcare team members, functioning as part of the nursing management team
Qualifications & Requirements:
- Must possess a current, unencumbered LPN/LVN license (as mandated by state regulations)
- Preferred: One or more years of direct nursing experience and familiarity with MDS/RAP
- Ability to utilize both local and consultant resources effectively in job execution
- Capability to develop a comprehensive understanding of state, federal, and regulatory requirements pertinent to the role
