We are looking for a dynamic self-starter to be our Healthcare Client Care Coordinator to join our award-winning Home Care agency.

  • Someone who will grow our client base, foster a "Team Approach" with care staff and maintain our high standards of client care.
  • Our ideal candidate is excited, energetic, dedicated, and a self-starter, as well as has direct client care experience and the ability to develop and motivate companions and staff members.
  • The role will be supported by the latest healthcare technology and best practices in the industry.

Key Responsibilities:

  • Serves as the primary point of contact for clients and their families, responding promptly to inquiries, concerns, and service requests.
  • Develop, implement, and evaluate programs to meet sales, marketing, financial, quality, and service goals.
  • Analyze and implement service opportunities, enhancing community engagement.
  • Conduct client care consults, assessments, and reassessments to ensure care meets client needs.
  • Assist in determining appropriate services and costs for clients.
  • Manage client records, service documentation, and invoicing.
  • Collaborate with the Scheduler to match caregivers with clients based on qualifications, availability, and preferences.
  • Regularly visit clients to assess quality of care and provide necessary supplies.
  • Monitor caregiver notes and family messages, responding to issues or changes in client needs.
  • Participate in client conferences and on-call rotations.
  • Fill in for caregivers as needed during business hours or while on-call.
  • Ensure all operations meet appropriate licensure and certification requirements.
  • Perform other administrative tasks as required.

Requirements:

  • 2+ years of direct client care experience, preferably in a home care setting, is required.
  • 2+ years of supervisory experience required.
  • 2+ years of administrative support experience required
  • Strong people management, interpersonal, and relationship-building skills.
  • Excellent organizational, attention to detail, and communication skills.
  • Ability to maintain professional boundaries and exercise sound judgment.
  • Valid driver’s license, safe vehicle, current auto insurance, and registration (vehicle stipend provided for work-related travel).
  • Daily local travel to client homes and working with field staff required.
  • Willingness to provide caregiving services, including personal care, homemaking, and dementia care, if needed.
  • Physical ability to perform duties, including walking, standing, and occasional lifting (up to 25 lbs)
