The role of the Case Manager is essential in ensuring the effective coordination of Community Long Term Services (CLTC) for designated participants, adhering to the guidelines set forth by SCDHHS/CLTC area offices. This position involves delivering comprehensive case management services to CLTC participants through both in-person and telephonic interactions during initial assessments, monthly check-ins, quarterly reviews, and annual evaluations.
Building strong relationships and collaborating with both public and private providers, as well as division leaders within FamilyTIES and SCDHHS, is crucial to delivering the most suitable and effective care for clients. Compliance with federal, state, and third-party payer regulations is mandatory in the provision of covered services, ensuring appropriateness and adherence to documentation requirements as specified by FamilyTIES and SCDHHS.
Key Responsibilities:
- Facilitate and coordinate care that is safe, timely, effective, efficient, equitable, and centered around the client.
- Manage case assignments, formulate service plans, monitor case progress, and determine case closure.
- Support clients in achieving wellness and independence.
- Oversee various aspects of care, including case coordination and information sharing.
- Act as an advocate for patients, helping them make informed decisions regarding their clinical status and treatment options.
- Establish and maintain effective working relationships with the medical team throughout the case management process.
- Document case information accurately, complete all necessary forms, and generate statistical reports.
- Encourage quality and cost-effective interventions and outcomes.
- Evaluate and address motivational and psychosocial challenges.
- Uphold professional standards as outlined by established protocols, rules, and regulations.
Required Skills:
- Demonstrated experience in case management, including nursing, medical, mental health, care management, or a related field.
- Thorough understanding of case management principles, healthcare management, and reimbursement processes.
- Experience with the psychological aspects of care.
- Strong communication skills.
- Exceptional organizational and time management abilities.
- Technical proficiency and professional skills.
- Problem-solving capabilities and the ability to manage multiple tasks simultaneously.
- Compassionate approach with strong teamwork skills.
Preferred Qualifications: A bachelor's degree from an accredited institution in a health or human services discipline that supports the physical, psychosocial, and/or vocational well-being of individuals served, along with documentation of at least two years of experience in providing case management services. Certification as a case manager is advantageous.
Job Type: Part-time
Compensation: $25.00-$30.00 per productivity
Schedule: Monday to Friday
