Position Overview

The role of the Case Manager is essential in ensuring the effective coordination of Community Long Term Services (CLTC) for designated participants, adhering to the guidelines set forth by SCDHHS/CLTC area offices. This position involves delivering CLTC case management services through both in-person and telephonic interactions during initial assessments, monthly check-ins, quarterly reviews, and annual evaluations.

Key Responsibilities:

  • Establish and maintain collaborative relationships with both public and private providers, as well as division leaders within FamilyTIES and SCDHHS, to guarantee that clients receive the most suitable and effective care.
  • Ensure compliance with federal, state, and third-party payer regulations while delivering covered services, confirming that all documentation requirements are fulfilled as per FamilyTIES and SCDHHS standards.
  • Conduct comprehensive assessments, develop strategic plans, implement necessary actions, monitor progress, and evaluate outcomes to meet the health and human service needs of clients.
  • Facilitate safe, timely, effective, efficient, equitable, and client-centered care coordination.
  • Manage case assignments, draft service plans, evaluate case progress, and determine appropriate case closure.
  • Support clients in achieving wellness and independence.
  • Coordinate various aspects of care, including case management and information sharing.
  • Act as an advocate for patients, assisting them in making informed decisions regarding their clinical status and treatment options.
  • Foster effective working relationships with the medical team throughout the case management process.
  • Document case information accurately, complete all necessary forms, and generate statistical reports.
  • Promote quality interventions and cost-effective outcomes.
  • Identify and address motivational and psychosocial challenges faced by clients.
  • Adhere to professional standards as outlined by relevant protocols, rules, and regulations.

Required Skills:

  • Demonstrated experience in case management, including nursing, medical, mental health, care management, or related fields.
  • In-depth knowledge of case management principles, healthcare management, and reimbursement processes.
  • Prior experience addressing psychological aspects of care.
  • Strong communication skills.
  • Exceptional organizational and time management abilities.
  • Technical proficiency and professional skills.
  • Problem-solving capabilities and the ability to manage multiple tasks effectively.
  • Compassionate approach with strong teamwork skills.

Preferred Qualifications: A bachelor’s degree from an accredited institution in a health or human services discipline that supports the physical, psychosocial, and/or vocational well-being of individuals served, along with a minimum of two years of experience in providing case management services. Certification as a case manager is advantageous.

Job Type: Part-time

Compensation: $25.00-$30.00 per productivity

Schedule: Monday to Friday
