Job Overview

As a Case Manager at FAMILYTIES OF SC LLC, you will play a vital role in coordinating Community Long Term Services (CLTC) for assigned participants. Your responsibilities will include delivering comprehensive case management services through both in-person and telephonic interactions during initial assessments, monthly check-ins, quarterly reviews, and annual evaluations.

Key Responsibilities:

  • Facilitate and ensure the delivery of care that is safe, timely, effective, efficient, equitable, and centered around the client’s needs.
  • Manage case assignments, develop service plans, monitor case progress, and determine case closure.
  • Support clients in achieving wellness and independence.
  • Coordinate various aspects of care, including case management and information sharing.
  • Act as an advocate for clients, assisting them in making informed decisions regarding their health status and treatment options.
  • Build and maintain effective working relationships with the medical team throughout the case management process.
  • Document case information accurately, complete necessary forms, and generate statistical reports.
  • Promote quality interventions and outcomes that are cost-effective.
  • Identify and address motivational and psychosocial challenges faced by clients.
  • Comply with professional standards as dictated by established protocols, rules, and regulations.

Required Skills:

  • Demonstrated experience in case management, particularly in nursing, medical, mental health, or related fields.
  • Strong understanding of case management principles, healthcare management, and reimbursement processes.
  • Experience with the psychological dimensions of care.
  • Exceptional communication abilities.
  • Outstanding organizational and time management skills.
  • Technical proficiency and professional skills.
  • Effective problem-solving capabilities and the ability to manage multiple tasks simultaneously.
  • Compassionate demeanor and strong teamwork orientation.

Preferred Qualifications: A bachelor’s degree from an accredited institution in a health or human services discipline that supports the physical, psychosocial, and/or vocational well-being of individuals. A minimum of two years of experience in providing case management services is also required. Certification as a case manager is advantageous.

Job Type: Part-time

Compensation: $25.00-$30.00 per productivity

Schedule: Monday to Friday
