Qualifications:

  • High School Diploma or equivalent education/experience.
  • Minimum of 1 year recent relevant experience in healthcare or care coordination.
  • Strong oral and written communication skills.
  • Excellent interpersonal and time management skills.
  • Ability to work effectively in a fast-paced environment with shifting priorities.
  • Ability to work independently with minimal supervision, while also collaborating effectively in a team.
  • Intermediate PC skills and proficiency in word processing and electronic medical records systems.

Job Responsibilities:

  1. Collaboration and Assessment:
    • Collaborate with Case Managers and Social Workers to conduct baseline patient assessments to identify post-hospital support needs and discharge requirements.
    • Jointly develop TOC plans, discussing choices for post-acute providers, available resources, and communicating expected discharge dates and arrangements.
    • Ensure patients and medical facilities receive information on benefit coverage, partnering with payers when necessary.
  2. Monitoring and Coordination:
    • Monitor progress towards TOC goals, promptly escalating barriers to Case Managers and Social Workers.
    • Keep patients and medical facilities informed about TOC plan progress throughout the hospital stay.
    • Coordinate post-discharge referrals and authorizations in collaboration with the Care Coordination (CC) team.
  3. Facilitation of Discharge Process:
    • Facilitate patient transfers to appropriate post-acute facilities, preparing necessary documents, obtaining physician signatures, and arranging transportation services.
  4. Departmental Goals and Objectives:
    • Participate in rounds with Case Managers and Social Workers to provide updates and receive direction on required assistance.
    • Deliver the Medicare “Important Message” (IM) to patients, informing them of their right to appeal discharge decisions.
    • Proactively identify and resolve barriers that may delay TOC plans, escalating unresolved issues to appropriate personnel.
  5. Communication and Documentation:
    • Maintain accurate and legible documentation according to department standards.
    • Enter CC notes in the electronic medical record to track referral statuses and communications.
    • Record patient preferences and key CC discussions in the electronic medical record.
    • Ensure timely closure of discharge cases in Allscripts after finalizing post-discharge providers.
  6. Customer Service:
    • Demonstrate tact and respect in all interactions with customers.
    • Build positive relationships with customers and partners through effective communication and issue resolution.
    • Provide and accept constructive feedback from colleagues and leaders to enhance service delivery.
    • Identify and escalate quality and risk management concerns to leadership as needed.
    • Ensure compliance with confidentiality policies and HIPAA regulations when transmitting patient information to external agencies or vendors.
