Candidates should possess knowledge and experience with Epic EHR

This role involves coordinating and executing the transition of care (TOC) and discharge (DC) plans for outpatient individuals. The specialist will prioritize and manage the care continuum through effective communication, critical thinking, and teamwork among healthcare providers, patients, families, and external partners to ensure timely patient discharge. The following are key responsibilities, requirements, and qualifications for this position:

KEY RESPONSIBILITIES:
COLLABORATION WITH CARE COORDINATION TEAM TO IMPLEMENT TRANSITION OF CARE PLAN.
  • Works closely with Case Managers and Social Workers to assess patient needs post-hospitalization and identify discharge requirements.
  • Engages with Case Managers and Social Workers to collaboratively develop the TOC plan, including discussing options for post-acute care providers and available resources.
  • Ensures that patients and healthcare facilities are informed about benefit coverage, collaborating with payers as necessary.
  • Monitors progress towards TOC objectives and alerts Case Managers and Social Workers to any obstacles encountered.
  • Keeps patients and medical facilities updated on the TOC plan throughout the hospital stay.
  • Coordinates all necessary post-discharge referrals and authorizations in partnership with the care coordination team.
  • Communicates regularly with Case Managers and Social Workers regarding the status of post-hospital provider referrals and any barriers to achieving TOC goals.
  • Facilitates patient transfers to appropriate post-acute facilities by preparing necessary documentation and assisting with transportation arrangements.
DEPARTMENTAL GOALS & OBJECTIVES.
  • Participates in rounds with Case Managers and Social Workers to provide updates and receive guidance on required assistance.
  • Delivers the Medicare "Important Message" (IM) and informs patients of their rights regarding discharge appeals.
  • Proactively identifies and resolves barriers to timely TOC plans, escalating unresolved issues to Case Managers and Social Workers or leadership.
  • Actively engages in daily team huddles and care coordination department meetings.
  • Contributes to team discussions on daily priorities, resolving conflicts, and creating actionable plans.
  • Collaborates with team members on interdependent tasks.
  • Demonstrates initiative and adaptability while working with interdisciplinary teams.
  • Shares knowledge and insights with team members to enhance team performance.
  • Builds and maintains trust-based relationships with colleagues and stakeholders.
COMMUNICATION.
  • Maintains accurate and current documentation in accordance with departmental standards.
  • Inputs care coordination notes into the electronic medical record to document referral statuses and communications.
  • Captures patient preferences and key discussions in the electronic medical record.
  • Ensures closure of discharge cases in Allscripts and documents final post-discharge provider information.
  • Provides clerical support as needed, including data entry and document management.
  • Completes all necessary forms for departmental reporting.
