SPARC Services and Programs is seeking a Care Management Supervisor. SPARC is dedicated to the mission of Keeping People out of Institutionalized Care and our Tailored Care Management (TCM) Program helps support our mission. SPARC provides behavioral health services to children, families, and adults across the Piedmont and Mountain Regions of NC.

We are looking for a dedicated Tailored Care Management (TCM) Supervisor for our TCM Program. This position is able to be based out of our Charlotte, Greensboro or Morganton offices.  

Qualifications & Experience: Supervising care managers serving members with behavioral health conditions must have the following minimum qualifications:

  • A license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession (including Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Addiction Specialist (LCAS), Licensed Clinical Mental Health Counselor (LCMHC), Licensed Psychological Associate (LPA)), or a Registered Nurse (RN) license issued by the North Carolina Board of Nursing;
  • and Three years of experience providing care management, case management, or care coordination to the population being served.

 

Primary Responsibilities-

The primary responsibilities of the TCM Supervisor are to provide clinical, administrative, and operational supervision and guidance to the Care Management team to promote whole person, integrated care. The TCM Program Manager plans, tracks, and monitors care to individuals to achieve the key goals of the behavioral health tailored plans.

The Behavioral Health and Intellectual/Developmental Disability (I/DD) Tailored Care Management model reflects the goal of whole-person care management in Medicaid Managed Care. Through TCM, clients will have a single designated care manager supported by a multidisciplinary care team to provide whole-person care management that addresses all of their needs, spanning: physical health, behavioral health, I/DD, traumatic brain injuries (TBI), pharmacy, long-term services and supports (LTSS) and unmet health-related resource needs.

Provision of  Care Management Supervisory Services-

  1. Supervision of Care Managers: One supervising care manager will not oversee more than eight care managers. Supervisors should have no caseload, but will provide coverage for vacation, sick leave, and staff turnovers.

 

  1. Development of Relationships with Clinical consultants. The clinical consultants (i.e. a Primary Care Doctor, Psychologist, etc) offer their expertise to support the needs of the panel.

 

 

  1. Oversight of Specific Care Management Functions: Ensure Care Management Staff are meeting expectations, requirements and timelines in these areas: Care Management Comprehensive Assessment; Care Plans and Individual Support Plans (ISPs) ; Development of the Care Team; Care Coordination; Twenty-four-Hour Coverage; Annual Physical Exam; Health Promotion; Addressing unmet health-related resource needs; Transitional care management; Diversion; and Ensuring the minimum contact requirements are met based on the members’ acuity

 

  1. Provide Oversight and Management to the Daily Operations of Care Management: Review and Approve Incident Reports; Review and Approve Progress Notes/other documentation; Review and analyze data ,reports, and trends in the EHR and Care Management Systems; Participate as a member of the agency’s leadership team; Develop and monitor implementation of SPARC Care Management Program; Embrace, demonstrate, and uphold the SPARC Values; Responsible for budget creation, implementation and oversight; Complete necessary documentation as required; Oversee the recruitment, hiring, and onboarding of staff. Ensure all pre-employment requirements are met o Provide initial onboarding/orientation and training with staff.   Recruit and interview applicants; Display competence and responsiveness to cultural differences present in both consumer and employee populations; Provide 24/7 crisis response to staff.
