Description:
Position Purpose:
Provide essential administrative support for care management, including outreach, call handling, scheduling, and documentation. Act as a key contact for members, providers, and staff to resolve issues and ensure compliance with state and regulatory guidelines.
Education/Experience:
- High School diploma or GED
- 1-2 years of related experience in a fast-paced call center
- 2+ years of healthcare experience and data entry
Key Responsibilities:
- Conduct member outreach and support care plan activities.
- Answer inbound calls and schedule services.
- Connect members with health plan and community resources.
- Document and maintain non-clinical member records.
- Support member onboarding and manage correspondence.
- Explain care plan procedures and protocols.
Required Skills:
- Strong communication and problem-solving skills
- Proficiency in MS Office (Excel, Word)
- Ability to adapt to change and handle a high-volume environment
Preferred Skills:
- Familiarity with medical terminology
- Typing

Entry-level
