Key Responsibilities of the LPN/RN MDS Coordinator:
  • Development of Resident Assessments: Lead and coordinate Resident Assessment Instruments (RAIs) in compliance with established guidelines.
  • Collaborative Care Planning: Partner with the Care Plan Team to design individualized care plans for residents.
  • Assessment and Reporting: Evaluate resident conditions, ensuring timely special assessments are performed, and provide essential data for the Quality Assurance Program.
Qualifications We Are Looking For:
  • Current LPN/RN License: Must be active and in good standing.
  • Experience as an MDS Coordinator: Previous experience in a similar capacity is required.
  • Knowledge of Regulations: Familiarity with Medicare/Medicaid guidelines and regulations is essential.
