ABOUT THE TEAM聽馃

The Supported Living (SL) team provides support for over 50 men and women, up to 24 hours a day, seven days a week if needed. Supports range from drop-in assistance with grocery shopping and bill paying to 24-hour support for those with more significant needs. The Personal Support Coordinator guides and coordinates the team鈥檚 efforts in assuring department goals and philosophy of fostering community belonging and building independence at home.

IN THIS ROLE, YOU'LL:聽馃摑

  1. Ensure the health, safety and well-being of all assigned supported individuals.
  2. Collaborate with family teams around community belonging and building independence in the family home.
  3. Effectively manage multiple projects and effectively prioritize them. Exercise good time management skills.
  4. Ensure that all items on the聽DSP Checklists聽and job requirements are completed according to the needs of the individual being served.
  5. Conduct individual and group staff meetings to discuss company policy updates, payroll and benefit changes, professional development etc.
  6. Maintain documentation on bi-weekly individual DSP supervision and weekly progress notes on supported individuals.
  7. Train all new staff in implementing individual Person Centered Plans and complete the necessary documentation in a timely manner.
  8. Coordinate and set up a smooth transition for all new supported individuals entering the Personal Supports Team to include: finding and interviewing potential Direct Support Professional for the individual, etc.
  9. Provide direct supports for the Direct Support Professional as needed for emergencies.
  10. Assist in any other function of SEEC as asked by the Supported Living Director/Director of Programs/ Executive Director.

ABOUT YOU聽馃

  • Bachelor鈥檚 degree is required
  • 5 years experience in related field
  • 2 years of supervisory experience, preferably within a residential or home setting.
