Description:
The **Clinical Pharmacist** position at our organization plays a crucial role in ensuring high-quality, patient-focused care and medication safety. Under the guidance of the Clinical Pharmacy Manager, you will be responsible for various aspects of Clinical Pharmacy, including providing pharmaceutical care, participating in education programs, and resolving medication-related issues. Your role will also involve developing and evaluating medication safety programs to enhance the quality of care.
Essential Functions and Responsibilities:
- Conduct Pharmacist duties on a rotating basis, offering pharmaceutical care across different age groups as per professional standards.
- Directly engage in patient care rounds, medication order reviews, and therapy monitoring to optimize medication usage.
- Collaborate with medical and nursing staff to identify patients requiring intensive pharmacy intervention and communicate care plans effectively.
- Monitor drug therapy, provide education, and assist in optimizing drug delivery.
- Develop and implement drug use protocols, contribute to medication safety, and evaluate medications for formulary considerations.
The **Clinical Pharmacist** will also lead quality improvement programs, educate healthcare professionals, stay updated on drug literature, and actively participate in research projects.

Required Education, Experience, and Certification/Licensure
Education:
- Doctor of Pharmacy Degree or clinical Master's Degree in hospital pharmacy from an accredited College of Pharmacy.
Experience:
- Minimum of two years of relevant experience, including a hospital pharmacy residency.
Licensure / Certification:
- State of Michigan Pharmacist license, controlled substance license, and Basic Life Support certification.
Required Skills and Abilities:
- Strong interpersonal and communication skills.
- Proficiency in pharmacokinetics and teaching abilities.
- Outstanding customer service orientation and research skills.
