Duties and Responsibilities

Assess the psychosocial status of patients and families/caregivers Conduct social evaluations and plan interventions accordingly Educate the team on specialized patient needs and cultural considerations Keep detailed clinical records for social work referrals Aid patients and families in advanced directives preparation Offer information and referral services for practical needs Act as a liaison between patients, families, and community agencies Collaborate with other healthcare personnel for patient support Develop relationships with external agencies for additional patient resources Participate in care plan development and interdisciplinary meetings Assist the team in recognizing psychosocial impacts on terminal illness symptoms Support quality assessment and performance improvement initiatives Help families and patients plan for end-of-life arrangements and responsibilities Perform other duties as assigned by the Clinical Supervisor

Requirements

Master's or Bachelor's degree in Social Work from an accredited program Minimum of one year experience in healthcare (hospice experience preferred) Understanding of hospice philosophy and end-of-life issues Excellent verbal and written communication skills Current licensure to practice in the relevant state according to Medicare guidelines Valid driver's license and insured automobile in good condition
