Description:

Position Summary:
This role involves coordinating and facilitating housing opportunities for individuals through various housing development activities, including housing stability case management, community integration supports, and advocacy for mainstream service supports based on participants' needs and eligibility.

Duties and Responsibilities:
  • Assist in completing applications for rental housing resources such as the Georgia Housing Voucher Program and HUD-funded programs
  • Conduct housing search activities, schedule interviews, and assist with the application process
  • Provide transportation and navigation assistance for documentation requirements
  • Support clients in meeting tenancy obligations and provide follow-up services for home maintenance
  • Offer referrals and care linkage for disability and employment services
  • Provide budget counseling and education for financial stability
  • Assist in developing individualized goal and action plans for permanent housing and self-sufficiency
  • Identify participant strengths and barriers to stability, provide resources and services linkage
  • Conduct pro-active home visits for stability and progress monitoring
  • Educate clients on residential lease contracts and maintain accurate records
Other Responsibilities:
  • Maintain working files and attend required meetings, supervision, and trainings
  • Submit incident reports in a timely manner
Qualifications:

Minimum Qualifications:

Bachelor's degree in Social Work, Public Administration, or related field. Equivalent experience is acceptable. Certified Peer Specialists are encouraged to apply. Must have a personal vehicle and valid Driver's License

Preferred Qualifications:

1-3 years of experience in homeless services or behavioral health field. Experience in housing services and community mental health services preferred. Familiarity with ClientTrack Homeless Management Information System and Coordinated Entry process. Passion for helping individuals experiencing homelessness is essential.
